All 4th and 5th grade students at GFJ must have a computer Acceptable Use Policy on file in the library.  This is mailed out with the new school year information.  If you need another copy or would like to have a copy of the document for your records, you may download the form here or pick up a copy at the library or in the main office. 

Parents and students must initial the bottom of each page as well as sign and date the final page.

STUDENTS MUST HAVE THIS SIGNED FORM ON FILE IN ORDER TO BE GIVEN A COMPUTER LOG IN PASSWORD.  Signed forms can be turned in to the classroom teacher or dropped off in the library. Thanks!
